Included in the Starter Pack:
- 1 care program (e.g., diabetes screening, hypertension monitoring, CVD risk follow-up)
- 2 primary user role (e.g., nurse, GP, specialist)
- 4-5 practitioner UI pages + care pathway navigation
- Care pathway orchestration to create a shared care plan with pre-defined care plan components
- Integration with FHIR endpoints to retrieve patient data and save shared care plan (See FHIR Foundation Pilot if the EHR system does not already support FHIR)
- Basic program reporting and operational dashboard
- Deployment + training
Not included in the Starter Pack:
- Patient-specific recommendations and alerts
- Multi-organization care network rollout
- Large-scale EHR integration to retrieve patient level data from underlying EHR systems that do not support agreed HL7 FHIR interfaces.
- Enabling single sign-on for healthcare practitioners
- Setting up a disease registry to enable structured collection of real-world clinical data during screening and monitoring encounters

Clinical Workflow Rollout (3–4 Months)
Achieve full integration with local care workflow, localization, get the platform ready for production, and implement full governance.
- Upon moving to production, we transition you to an Annual Subscription.
- This license covers both the software engines and the intellectual property (mappings/schemas) developed during the pilot.
- The “Success Credit”: Organizations that sign a 2-year Annual Subscription agreement following a successful Pilot and Rollout phases receive a credit of 50% of their pilot investment toward the first year’s license.

Add guideline-based recommendations and SMART/CDS integration for creating personalized care plans and governed rollout.
