Integrated care platform mode
KronIQ enables care teams to coordinate longitudinal care through shared care plans and structured workflows, supporting consistent follow-up across roles, settings, and organizations.
What it does
- Enables creation and coordination of shared care plans using FHIR CarePlan and related resources (e.g. ServiceRequest, Goal, orders), aligning work across the care team
- Provides role-specific user interfaces for GPs, nurses, specialists, and case managers
- Supports cross-organization workflows where governance and integration allow, with consistent documentation and structured data capture
- Focuses on execution and follow-up (tasks, timing, responsibilities), rather than making clinical recommendations
- Supports standardized care pathways for screening and monitoring of selected diseases
- Allows care pathways to be defined and executed as guided workflows through configurable encounter flows
- Guides practitioners through encounters using page-based care flows, showing only semantically and temporally relevant information
- Uses specialized UI components (e.g. timelines, comorbidity views, calculated indicators) to reduce cognitive load and avoid raw data lists

Yes. Many organizations start with coordination, tasking, and structured workflow guidance first, then add CDS later if desired.
If we add decision support later, do we need to redesign pathways?
No. The pathway model can stay the same; decision support is added as an optional module that enriches specific steps.
